Worms coming out of Soil

: I noticed that there are small caterpillar looking worms crawling on the soil of my plants and they sometimes crawl out of the pot and onto the floor. I have a 9mth old and don't want him to get a hold of one of these. How do I get rid of these nasty worms? Also, it looks like the soil is growing mold of some sort. PLease help. Thank you.
