The remaining phal you have now, how long have you had it? about 1 month or so.

You say it is "going strong"? Does that mean it is growing a new leaf? i mean that the leaves are nice and green and until recently the stems were good too. but there were little mini buds that were not fully even buds yet. And i did not see them growing at all. then i noticed they seemed to be drying out. that is when i decided to cut at the nodes on each stem and that was a big mistake b/c now the each stem is yellow at teh tips and beginning to dry out.

How often are you watering? i water once a week. i drench it in water, let the water run out and i do that every thursday.

Is it in a window? it is near a window, but not in direct sun light.

Do you use A/C or fans with open windows for summer cooling? i use an AC. it's not directly in front of the AC tho.

Where do you live? East, west, north, or south facing window? NYC, and the window faces East.

Can you tell what it is potted in? Is it in bark pieces and if so, how big are the pieces? seems to be in moss. i knmow moss is usually decorative but i stuck my finger in and it seems to be moss all the way down.

Where did you purchase said plant? a flower store here in Manhattan. It was a BEAUTIFUL plant!!! so many flowers and buds. Clearly had been taken care of very well (until i got my hands on it) it has about 4 full stems growing from one original stem.
How often do you water and are you using any fertilizer? Are you using tap water, rain water, water from a water softener (baaaad thing to do to an orchid)? water with tap once a week. no fertilizer
