can I move my perenial bed to a new house?

I'm movimg to a new house, and I'd like to take some of my perenials with me, but I'm not sure if they'll survive the move or not. I have "summer wine" day lillies, lavender "bushes", some beautiful blue larkspur that bloomed forever this summer,and some kind of orange lillies that have small pointed leaves running all the way up the stems..can I move them in September- October... in Ohio?( and if so,how do I give them the best chance of surviving)? Any imput would be much appreciated...
