identify perennial

I am looking for the name of a small perennial. The leaves grow right out of the soil and are medium green, oval (about an inch long and slightly serrated). It forms a small clump 1 or 2 inches high. The flowers are small yellow pouches (half an inch)on single stems about 3 or 4 inches tall. Semi shade and probably zone 3 or 4. The greenhouse I orginally bought it from called it a "slipper flower". It is not one of the orchids.
