There could be several reasons, some of which could be drought (not being watered enough when first planted), root rot, deficiencies in the soil, etc.
I wouldn't suggest using a fertilizer until you find out why the plant is failing. You could do a soil test too. Take a look at this site. Click on 'Broadleafed Shrubs' on the left and look first at the leaves. There are several possibilities. If there are any words that you don't understand, go to
